The Stroke Checklist project aim is to develop and disseminate actionable, evidence-based stroke care checklists relevant to a global audience in their everyday clinical practice. These checklists address a variety of stroke-related topics ranging from hyperacute thrombolysis and endovascular care to secondary prevention management and rehabilitation. With an emphasis on supportive management, these checklists are deigned to be adaptable to different local resource settings. They are freely accessible through the WSA platform to stroke providers worldwide to use to ensure adherence to best practice, reduce variability in care and improve stroke outcomes.
This Post-Stroke Checklist (PSC) has been developed to help healthcare professionals identify post-stroke problems amenable to treatment and/or referral. The checklist is available in American English, British English, Austrian, Canadian French, German, Portuguese, Spanish and Swedish.

This guideline is tailored for healthcare professionals in the in-hospital stroke patient care at both essential and advanced stroke centers. The format of this guideline prioritizes actionable, evidence-based recommendations. Hospitals aiming to implement this checklist must meet specific prerequisites, including access to a CT scan, laboratory facilities, a well-equipped Emergency Department, an ICU, the potential for acute thrombolytic therapy and endovascular treatment and a demonstrated proficiency in stroke care. For healthcare facilities with a lower level of stroke care, immediate transfer of suspected stroke patients to the nearest stroke-ready hospital is recommended.
